Incident Accident Investigation

The cost of an incident accident at work is high and frequently higher than understood. For every £1 an incident causes in direct costs (repairs, absence staff, etc) there is approximately £15 in indirect costs, such as higher insurance premiums, loss of business etc. T&OD Org's annual support contract includes, as standard, an accident investigation service at no additional fee -minimising the initial cost to your business should an accident occur.

Our in-house health and safety consultants will respond rapidly to get your business up and running in the event of an accident or an improvement or prohibition notice. We will aim to identify and analyse the relevant issues and make recommendations aimed at preventing a re-occurrence.

We can also assist you with the reporting of health and safety incidents under the Report of Injuries, Diseases and Dangerous Occurrences Regulations 1995 (RIDDOR) as well as liaising with enforcing authorities on your behalf.

You can be assured that, in all cases, our consultants are diligent in the sensitivity that an accident investigation requires and will act in a responsible, professional manner. Pending agreement of specific arrangements, a guide for our standard procedure for accident investigation would be:

* Advise on actions to deal with immediate risks;
* Record details about the event;
* Check on the condition of any plant, machinery, equipment or chemicals - this may involve specialist advice;
* Interview witnesses;
* Interview injured persons when appropriate to do so;
* Check on any previous accidents/incidents in order to identify trends;
* Review training records;
* Complete the incident accident illness report form and prepare a written report;
* When the report is complete take sufficient copies and circulate to the appropriate persons;
* Review risk assessments, safe systems of work, training etc to prevent a re-occurrence;
